Since 2018, the Northwestern Medicine Collaborative Behavioral Health Program (CBHP) has provided mental health care to patients across Chicagoland. The implementation of CBHP has led our team to significant learnings of collaborative care integration, which have been published across a variety of medical and academic journals.
